Breaking Barriers in Diabetes Prevention: Amistad’s Inclusive Approach Leads the Way in Texas

At the forefront of community health innovation, Amistad is making a difference for some of the Paso del Norte region’s most vulnerable populations as a nonprofit organization dedicated to serving older adults, people with disabilities, and those at risk.

With support from the Health Foundation’s Disease Prevention and Management Initiative, Amistad launched the Diabetes Awareness Prevention Team (ADAPT) — a first-of-its-kind program in Texas designed to prevent Type 2 diabetes with full inclusion of people with disabilities. By removing barriers to participation and providing flexible options—online, in-person, and hybrid—ADAPT makes it easier for individuals and families to take control of their health.

The program currently supports 145 participants who receive quarterly incentives to stay motivated and engaged. Its inclusive design also draws in caregivers, parents, and extended family members, expanding the program’s reach and impact. Through Amistad’s diverse client base, they reach underserved populations and connect them with valuable diabetes prevention resources in partnership with the Health Foundation. Participants in the ADAPT Program often encourage their loved ones to join them, fostering a supportive environment where they can motivate each other and stay accountable.
