2021 Diabetes Education Call for Letters of Intent (LOI)

The Paso del Norte Health Foundation (Health Foundation) seeks Letters of Intent (LOIs) from eligible organizations to provide diabetes education.
This LOI is specific to diabetes education interventions. Therefore, any intervention with the primary aim to expand reach of or enrollment in diabetes, including pre-diabetes, education is eligible. This may include becoming recognized (ADA or AADE) as a provider of diabetes education, providing the Diabetes Prevention Program, offering diabetes education using new technology (telehealth), testing various referral or marketing models for diabetes education, designing new models for diabetes education program delivery, strengthening diabetes education reimbursement systems, educating hard to reach populations, adopting best practice education programs related to diabetes, and offering diabetes education at worksites. Also included in this RFP is opportunity for educating caregivers and family members of people with diabetes. Interventions to improve diabetes education quality will be considered.
Organizations are encouraged to use existing evidence-based diabetes educational materials, curriculum, media, or other such products.
The Health Foundation is considering the release of another RFP emphasizing organizational and professional capacity building. Proposals under this potential second RFP may advance professional preparation, continuing education, technology in diabetes detection, tracking, or management, compliance with ADA Standards of Medical Care, service delivery systems, co-locating programs, and public policy.
The Health Foundation uses LOIs to welcome ideas from a diverse applicant pool. After review, the Health Foundation will invite selected applicants to submit a full proposal. Selected applicants may be asked to address specific comments or questions from the LOI in their full proposal.

Eligibility And Selection Criteria
Eligible applicants include nonprofit tax-exempt organizations, including churches and units of government, located in the Paso del Norte region. See the Grant Center on the Paso del Norte Health Foundation website www.pdnhf.org for more information about grant guidelines.
Service Region
The Paso del Norte Health Foundation operates in a unique binational environment, which stretches across two countries and three states. This region serves the 2.4 million people living in El Paso and Hudspeth Counties in far west Texas, Doña Ana, Luna, and Otero Counties in southern New Mexico, and the municipality of Ciudad Juárez, Chihuahua, Mexico.
